Please provide 10 readable signed copies of all application material (photocopies acceptable).
Receipt of your application will be confirmed by letter.
You will be informed of the Trust’s decision approximately two months after the close off date for applications.
State the name of the individual or organisation applying for funding. This is the name of the individual or organisation, if the application is successful, which will invoice the Trust.

State the address to which you want all Trust information sent.

Provide details of where the activities of the project are to take place e.g. nationwide, province, city, and town. If applicable, indicate if the location is changing from past years.
State the aims and provide specific details of the project. The following example is provided as an illustration of an outcome/goal:
"Project: provide driver licence theory course for long term unlicenced drivers. Outcome/goal: deliver five driver theory courses to verifiable long term unlicenced drivers achieving an overall pass rate of at least 98%"
State the measurable benefit/s to road safety from the project referring to the Government's road safety priorities as outlined in the Road Safety to 2010 strategy State also how the project will be consistent with the New Zealand Transport Strategy insofar as it relates to road safety.
Give us information that will enable the Trust to assess, if supported, that the project and its budget will be efficiently and responsibly managed. Provide a detailed work or action plan for the project that clearly shows the project stages together with the project start and completion dates. The anticipated first grant claim date is also to be provided.
Note: the level of detail of the application should relate to the amount of money the applicant is seeking.
State how the project will be monitored, indicating target dates by which specific outcomes/goals will be achieved.
State how the project will be evaluated in terms of achieving its outcomes/goals and providing value for money or provide details if you are seeking funding for independent evaluation.
NB If you claim funding for a formal evaluation of the project you will need to show how this is built into the project plan and provide the name of the evaluator and their accredited qualification.
State the intended frequency of project reporting to the Trust.
Tell us what money you already have for the project and how the rest of the money will be obtained. Include a breakdown of how the money will be used and state projected income if applicable.
Note: to assist funding decisions a detailed breakdown of costs must be provided.
Attach the latest audited Balance Sheet and Income and Expenditure statements from your Annual Accounts.
Please provide a copy of your Annual Report.
Note: these are only necessary if the application is from a company or organisation.
If you are registered for GST you are entitled to claim the amount sought as funding including GST.
Give the full name of each Trust from which the project/company/organisation already receives funds and the amounts of each of these.
If this is an ongoing project state the usual source of funding for the project.
Provide the name and contact details of organisations or individuals from whom you have received funding recently.
State the type of organisation e.g. charitable trust/other. Please provide also a copy of your organisations Certificate of Incorporation and the names of all office holders and trustees.

State the qualifications, accredited or otherwise, and experience relevant to the delivery of this project.
Please provide details and attach a signed and completed copy of the form PV/F2: (Request by a Third party under the Official Information Act 1982 for a copy of an Individuals Criminal Convictions held on the Ministry of Justice’s Computer Systems) (PDF, 64 KB) authorising the Trust to access all your Ministry of Justice records.
NB: Only those whose applications are successful will have their Ministry of Justice records accessed by the Trust.
